Welcome to Kheiron Retina, if you are scheduled for an appointment, we look forward to meeting you.

Prior to your scheduled appointment, please scroll below to our "New Patient Forms" tabs and then download and complete the appropriate patient registration form and bring it with you on the day of your appointment.

Please bring your Identification Card (ID), as well as the most current health insurance cards with you on the day of your appointment. If you have an insurance plan that requires a referral, please call your primary care provider (PCP) or the referring ophthalmologist (EyeMD) and have them initiate a referral to Kheiron Retina. The referral must be current for the date of your appointment here.

If your insurance requires you to pay a co-payment, this will be due at the time of service. Please be advised that a recommendation from your primary care physician (PCP), referring EyeMD or optometrist (OD) to see a Kheiron Retina provider does not guarantee payment by your insurance plan. If you have any questions regarding your health insurance coverage, please contact your health insurance plan prior to your visit.

Please be advised that an initial appointment could take up to 2 hours. We welcome you to our practice. Please feel free to contact the office with any questions you may have.
